Abstract

BACKGROUND::

The gold standard for treatment of septic arthritis is urgent surgical debridement. Preoperative magnetic resonance imaging (MRI) may identify osteomyelitis, subperiosteal abscesses, and intramuscular abscesses, which frequently occur with septic arthritis. If these adjacent infections are not recognized, initial treatment may be inadequate. The purpose of this study is to develop a prediction algorithm to distinguish septic arthritis with adjacent infections from isolated septic arthritis to determine which patients should undergo preoperative MRI.

METHODS::

An IRB-approved retrospective review of 87 children treated for septic arthritis was performed. All patients underwent MRI. Sixteen variables (age, sex, temperature, WBC, CRP, ESR, ANC, hematocrit, platelet count, heart rate, systolic blood pressure, diastolic blood pressure, symptom duration, weight-bearing status, prior antibiotic therapy, and prior hospitalization) from admission were reviewed. Graphical and logistical regression analysis was used to determine variables independently predictive of adjacent infection. Optimal cutoff values were determined for each variable and a prediction algorithm was created. Finally, the model was applied to our patient database and each patient with isolated septic arthritis or adjacent infection was stratified based upon the number of positive predictive factors.

RESULTS::

A total of 36 (41%) patients had isolated septic arthritis and 51 (59%) had septic arthritis with adjacent foci. Five variables (age above 3.6 y, CRP>13.8 mg/L, duration of symptoms >3 d, platelets <314×10 cells/μL, and ANC>8.6×10 cells/μL) were found to be predictive of adjacent infection and were included in the algorithm. Patients with ≥3 risk factors were classified as high risk for septic arthritis with adjacent infection (sensitivity: 90%, specificity: 67%, positive predictive value: 80%, negative predictive value: 83%).

CONCLUSIONS::

Age, CRP, duration of symptoms, platelet count, and ANC were predictive of adjacent infections. Patients who met ≥3 criteria are at high risk for adjacent infection and may benefit from preoperative MRI.

ANTECEDENTES :

El estándar de oro para el tratamiento de la artritis séptica es el desbridamiento quirúrgico urgente. La resonancia magnética preoperatoria (IRM) puede identificar la osteomielitis, abscesos subperiósticos y abscesos intramusculares, que se producen con frecuencia con artritis séptica. Si no se reconocen estas infecciones adyacentes, el tratamiento inicial puede ser inadecuada. El propósito de este estudio es desarrollar un algoritmo de predicción para distinguir la artritis séptica con infecciones adyacentes de aislado artritis séptica para determinar qué pacientes deben ser sometidos a resonancia magnética preoperatoria.
MÉTODOS :
Se realizó una revisión retrospectiva aprobado por la IRB de 87 niños tratados por artritis séptica. Todos los pacientes fueron sometidos a resonancia magnética. Dieciséis las variables (edad, sexo, la temperatura, del CMB, PCR, VSG, ANC, hematocrito, recuento de plaquetas, la frecuencia cardíaca, la presión arterial sistólica, presión arterial diastólica, duración de los síntomas, el estado de carga de peso, tratamiento antibiótico previo, y la hospitalización previa) admisión fueron revisados. Se utilizó el análisis gráfico y logístico de regresión para determinar las variables independientemente predictivos de infección adyacente. Los valores óptimos de corte se determinaron para cada variable y un algoritmo de predicción fue creado. Finalmente, el modelo se aplicó a nuestra base de datos de paciente y cada paciente con artritis séptica aislado o infección adyacente se estratificó basa en el número de factores predictivos positivos.
RESULTADOS :
Un total de 36 (41%) de los pacientes había aislado artritis séptica y 51 (59%) tenían artritis séptica con focos adyacentes. Se encontraron cinco variables (edad por encima de 3,6 y, CRP> 13,8 mg / l, duración de los síntomas> 3 d, plaquetas <314 × 10 células / l, y RAN> 8,6 x 10 células / l) para ser predictivo de la infección adyacente y fueron incluidas en el algoritmo. Los pacientes con ≥3 factores de riesgo fueron clasificados como de alto riesgo para la artritis séptica con infección adyacente (sensibilidad: 90%, especificidad: 67%, valor predictivo positivo: 80%, valor predictivo negativo: 83%).

Conclusiones :
La edad, el CRP, duración de los síntomas, el recuento de plaquetas, y el ANC fueron predictivos de infecciones adyacentes. Los pacientes que cumplían ≥3 criterios están en alto riesgo de infección adyacente y pueden beneficiarse de la RM preoperatoria.

Abstract

This study assessed the presentations and outcomes of patients treated for Iselin's apophysitis. A total of 27 patients with Iselin's apophysitis were retrospectively analyzed for presentation, treatment, and clinical outcome. The mean age of presentation was 11.5 years (range: 9.2-15 years). All patients played sports, and their symptomatic severity and acuity varied. Treatments included rest, footwear modification, use of a controlled ankle movement boot, and use of a post-op shoe. Treatment lasted for an average of 24 days (range: 14-33 days), and the average time to return to play was 38 days (range: 14-53 days). All cases healed, with three reinjuries and no nonunions. Six cases were initially misdiagnosed as a fracture or tendonitis, indicating the need for further education on the diagnosis and natural history of Iselin's apophysitis.

Este estudio evaluó las presentaciones y los resultados de los pacientes tratados porapophysitis de Iselin. Un total de 27 pacientes con apophysitis de Iselin se analizaronretrospectivamente para la presentación, tratamiento y evolución clínica. La edad media de presentación fue de 11,5 años (rango: 9,2-15 años). Todos los pacientes jugabandeportes, y su severidad sintomática y la agudeza variaron. Los tratamientos incluyerondescanso, el cambio de calzado, el uso de una bota de movimiento del tobillo controlada,y el uso de un zapato post-op. El tratamiento duró un promedio de 24 días (rango: 14 a 33 días), y el tiempo medio para volver a la reproducción fue de 38 días (rango: 14-53días). Todos los casos curados, con tres recidivas de lesión y sin pseudoartrosis. Seis casos fueron mal diagnosticados inicialmente como una fractura o tendinitis, lo que indica lanecesidad de una mayor educación sobre el diagnóstico y la historia natural de la apofisitis de Iselin´s.

Abstract


The differentiation of physiological pediatric flat feet and the pathological course as well as adequate initiation of therapeutic options represents a challenge the importance of which should not be underestimated. Fears and worries of parents and their insistence on inducement of therapeutic measures take center stage in the physician-patient relationship. The problem is that there are insufficient data from scientific studies dealing with the conservative treatment of flat feet in children. This paper intends to give a survey of the current status in the literature concerning indications and therapeutic success using insoles in the treatment of flat feet in children.

La diferenciación de los pies planos pediatricos fisiologicos del curso patológico, así como la iniciación adecuada de opciones terapéuticas representa un desafío cuya importanciano debe ser subestimada. Los miedos y las preocupaciones de los padres y su insistencia en la inducción de medidas terapéuticas toman protagonismo en la relación médico-paciente. El problema es que no hay suficientes datos de estudios científicos sobre el tratamiento conservador del pie plano en los niños. Este documento tiene la intención de dar un estudio de la situación actual en la literatura acerca de las indicaciones y de éxito usando plantillas terapéuticas en el tratamiento del pie plano en los niños.

Abstract

BACKGROUND:

Recently, many studies have argued against the Kidner procedure and its several modifications, with the accessory navicular considered an irritant rather than a hindrance to foot function, and simple excision has been advocated. We designed this prospective, comparative study of simple excision and the Kidner procedure in children and adolescents with flatfoot and evaluated functional and radiographic outcomes.
METHODS:

From February 2004 to March 2009, we followed 50 consecutive feet of symptomatic type 2 accessory navicular. Simple excision and the Kidner procedure were performed in 25 feet each, respectively. The American Orthopaedic Foot and Ankle Society (AOFAS) Midfoot Scale, the visual analogue scale (VAS), and the talo-first metatarsal, talocalcaneal, and calcaneal pitch angles were evaluated preoperatively and postoperatively for a minimum of 3 years. Patient satisfaction was assessed.
RESULTS:

The preoperative AOFAS midfoot scale and VAS of each group were improved at final follow-up. The talo-first metatarsal and talocalcaneal angles were not significantly different pre- and postoperatively. However, the calcaneal pitch angle of both groups was improved at the final follow-up. At the final follow-up, we observed no significant differences between the 2 groups in the AOFAS midfoot scale, the VAS, and the talo-first metatarsal, talocalcaneal, and calcaneal pitch angles. The satisfaction frequencies were 86% and 82% in groups 1 and 2, respectively.
CONCLUSIONS:

We found that in children and adolescents with accessory navicular and flatfoot, simple excision and the Kidner procedure both gave satisfactory results and both minimally restored the medial longitudinal arch similarly.

ANTECEDENTES:

Recientemente, muchos estudios han argumentado contra el procedimiento de Kidner y sus varias modificaciones, con el  accesorio 
navicular considerado un irritante en lugar de un obstáculo para la función del pie, y la escisión simple se ha recomendado. Diseñamos este estudio prospectivo, comparativo de la escisión simple y el procedimiento Kidner en niños y adolescentes con pie plano y los resultados funcionales y radiográficos evaluados.

MÉTODOS:
Desde febrero 2004 hasta marzo 2009, seguimos 50 pies consecutivos del accesorio navicula  sintomático tipo 2 . Escisión simple y el procedimiento Kidner se realizaron en 25 pies cada uno, respectivamente. La American Orthopaedic Foot and Ankle Sociedad (AOFAS) Midfoot escala, la escala visual analógica (EVA), y el metatarso-talo primero, talocalcaneal, y ángulos de inclinación del calcáneo fueron evaluados pre y posoperatoria para un mínimo de 3 años. La satisfacción del paciente se evaluó.
RESULTADOS:
 Al final del seguimiento, no se observaron diferencias significativas entre los 2 grupos en la escala AOFAS el medio pie, la EVA, y los ángulos metatarsiano, talocalcaneal, y de paso calcáneo-talo primero. Las frecuencias de satisfacción fueron 86% y 82% en los grupos 1 y 2, respectivamente.
CONCLUSIONES:
Se encontró que en los niños y adolescentes con  accesorio
navicular  y pie plano, la escisión simple y el procedimiento Kidner tanto dieron resultados satisfactorios y  mínimamente se restauró el arco longitudinal medial de manera similar.
